Greenbriar Community Care Center is part of the CommCare family, a Louisiana non-profit corporation which operates 15 skilled nursing facilities all in the state of Louisiana.  We believe that our company is small enough to recognize and support individual needs of each center, yet large enough to bring the resources necessary to continuously improve quality of care.

Culture Change

“Culture change” is a movement that seeks to transform services for older adults based on person-directed values and practices. This philosophy considers and respects voices of elders and upends the traditional practices of providing care. The design of the building puts residents in the center of the households, rather than the staff as has been the practice with a nurses; station at the center of traditional operations. Each household has its own entrance, laundry, kitchen and outdoors space. Common areas include a salon, cafés where residents and families can help themselves to coffee, beverages and soft serve ice cream.

Outdoor spaces include intimate porches, courtyards and verandas as well as a spacious green space leading up to our center.

Beyond the building, our staff is on a journey to learn new ways of providing care from understanding what residents desire from when they arise and retire to when they bathe. Meal times and meal choices will take on a different look and feel. Consistent staffing in the households are designed to create a sense of family and community where caregivers really get to know residents and their families.

Core values are choice, dignity, respect, self-determination and purposeful living.

Welcome to Greenbriar Community Care Center!

We thank you for visiting our website and invite you to visit us at Greenbriar Community Care Center. We are dedicated to providing quality care with an enriching experience. In fact, each year families and residents complete a satisfaction survey through a company that surveys more than 30,000 skilled nursing and rehabilitation centers nationwide. We are proud to share our results. 100% of families and residents ranked their overall satisfaction level as good or excellent. We believe that this level of satisfaction begins with tenured staff who really feel like residents are their extended families.

Secondly our staff includes experienced personnel including nurses with certifications in long term care, wound care and IV therapy. We have engaged a nurse practitioner who brings another level of expertise to our center. In addition to the education and experience of our staff, Greenbriar Community Care Center focuses on monthly continuing education topics and staff members have access to online education 24 hours a day, seven days a week.

Our varied activities and events bring a vibrant and exciting atmosphere to Greenbriar Community Care Center. Visitors and volunteers create a warm, home-like environment.

Additionally, our staff is taking the journey to the philosophy of Person-centered care, which means that we provide care in an environment that respects the desires of the individual. So we look for opportunities to offer choice – whether it is offering choices at mealtimes, a refreshment bar where residents can help themselves at any time or choice of bathing times. We honor resident preferences such as natural awakening, which means residents awaken when they choose, rather than for our convenience.

The emphasis of our Rehab Community is on rehab to home, where a wing of our center is dedicated to help individuals attain this goal. The emphasis on return to home creates an atmosphere where everyone encourages one another. In fact, while we still provide traditional long term care, 61.8% of those who turn to us for care, rehab and return home successfully.
